Please be aware that this office requires 24 hours notice to change or cancel an appointment. If you do not show up for your appointment or give us less than 24 hours notice , you will be charged the full cost of the appointment

While you do not require a doctor's referral to make an appointment, some extended health plans do require one for reimbursement. These plans vary greatly in coverage and it is therefore beneficial to contact your provider for details. A receipt will be issued to you for reimbursement.

We are able to direct bill for the following insurance providers. Please bring your policy identification numbers if you would like us to do so. You are still responsible for any portion not covered by your plan. Please note, we are only able to bill these insurance providers if you are already set up for electronic billing.
